It will save you waiting time if you have these documents filled out prior to your first office visit with us.  Please do not sign anything that you do not understand. We will fully explain to you what you don't understand on the documents when you arrive at our office for your appointment.

After completing the forms, please attach all 3 documents together and bring them with you to your appointment.
